Rapid Assessment, Correction & Assurance of data quality on RTT waiting lists

Imperial College Healthcare NHS Trust instructed us to address data reporting issues on their RTT waiting lists – with the majority focused on improving data quality. The Trust also wanted to make sustainable improvements to their processes and accountability frameworks to ensure their data was accurate and all patients were safely tracked.

OUR SOLUTION

·  Setup a Programme Management Office with a lead on designing and delivering the comprehensive data quality improvement plan.
·  Developed and embedded clear performance management frameworks/governance structures with regular reporting to internal and external executives (and regulators).
·  Provided highly skilled validators to deliver rapid correction of at-risk pathways using our sampling methodology, which delivered a fast-paced resolution.
·  Carried out root cause analysis of all data quality issues leading to associated sustainable improvement and training recommendations.

THE OUTCOMES

· Validated 40,000 pathways
· Reviewed 320,000 pathways
· Identified 88% of pathways as not requiring validation
· Completed all key recovery tasks in less than a year.

Elective Orthopaedic Centre Development

In early 2022, the four acute trusts operating under the North West London ICS umbrella came together to propose a new way of organising planned orthopaedic (bone and joint) inpatient surgery for adults. We were commissioned to conduct a comprehensive predictive modelling exercise to assess the impact of the proposed new model of orthopaedic care, as well as supporting in a number of other ways.

Our solution

·  Carried out waiting list forecast modelling, drawing on multiple datasets. 
·  Conducted waiting times predictions, using queuing methodology to provide estimation of future waiting times based on service demand and target admission waiting times. 
·  Evaluated clinical and operational models, ascertaining the best service configuration to generate the greatest benefits for patients 
·  Provided general advisory services to the programme’s steering group, including data quality considerations and materiality/sensitivity matters.

The outcomes
·  Created more than 12 modelling scenarios
·  Analysed the care of 16,000 orthopaedic patients
·  Reduced waiting list size by 30% and an 8 week reduction in waiting times evidenced

Medicine Division Support

Surrey and Sussex Healthcare NHS Trust commissioned us to support the Medicine Division over the busy winter period, with our Director of Consulting, Stephen Hall, delivering a project to help stabilise and then improve structures, processes and ultimately the patient experience.

OUR SOLUTION

·  Implemented a clear process, with timelines, for job planning within the Medicine Division.
·  Reviewed and put in place a robust process for the validation of all ED breaches and ambulance handover breaches.
·  Introduced and chaired twice weekly Long Length of Stay (LLoS) and No Criteria to Reside(CtR) patient-by-patient reviews within Medicine, to support expediting safe, timely discharge.
·  Implemented robust Christmas and New Year contingency plans, including Triumvirate-focused patient ward rounds, additional consultant ward rounds at the weekend/bank holiday, and the daily presence in ED of the Director of Operations & ChiefNurse

Helped to reduce the financial run rate in the Medicine Division compared to the same period the year before – with savings of £2.3m
·  Saved circa £170k on locum consultants by writing a successful business case to increase the substantive consultant Whole Time     Equivalent (WTE) in General Internal Medicine
·  Wrote a successful business case to increase the number of Clinical Directors from 2 to 4 Improved ED 4-hour performance by 11% despite a 16% increase in the number of attendances
·  Led the reconfiguration of the medicine bed base twice in a four-month period, to ease pressure on wards.
